Opening Promotion Terms & Conditions

1. Information on how to participate forms part of these Terms & Conditions. By participating, claimants agree to be bound by these Terms & Conditions. Claimants must comply with these Terms & Conditions for a discount to be valid.
2. Each claimant is entitled to a discount of 30% when they attend a session at Head 2 Toe Kids & Family Health. The discount is not transferable and are not redeemable for cash.
3. To redeem the discount:
   • the discount will automatically be added to current client’s invoices during the month of March 2019.
   • New clients can access the 30% discount during the month of March if they meet the access requirement.
4. Access requirement:
   • In order to access the 30% discount, the parents/caregiver must have true concerns about the child’s communication, speech, fluency, voice, literacy, feeding, language, behaviour, fine motor skills, gross motor skills, development, learning, functional skills and/or cognition. New referrals will be screened over the phone for appropriateness and an application form must be filled out prior to accessing services. This will determine if there is true concern and need for the chosen service. If true concern is met, the family can access the 30% discount. If the claimant fails to identify concern the discount will not be applied. Families will be notified of this prior to accessing the service.
5. The 30% discount is valid for a limited time only and expires on the 31^st March 2019
6. After payment has been received families will be provided with a receipt stating the discount amount.
7. Head 2 Toe Kids & Family Health will seek payment for their provision of supports after the participant’s representative attends their appointment. You will be required to settle your account in full at the end of your appointment at the clinic. Payment can be made using cash, EFTPOS, Visa or MasterCard.
   • An invoice for non-clinic-based services will be provided upon their completion and payment should be made via direct deposit within 48 hours of receiving the invoice.
   • Head 2 Toe Kids & Family Health can process private health fund and NDIS claims at the clinic upon request. You will be required to pay any gap between the fee charged and your health fund rebate.
   • If an appointment is not paid for no further appointments will be made until payment is received.
8. Therapy will continue on an ongoing basis until either:
   • Your therapist discharges your child as they no longer require therapy
   • You contact Head 2 Toe Kids & Family Health to give notice of discontinuing therapy
9. This is an obligation free offer.
